Honoring Wishes Across Care Settings: The Role of Palliative Care in POLST Export to iCal/Outlook Print

This audio conference will examine the effectiveness of the POLST Paradigm program including cutting-edge innovations and data about reduced, unwanted hospitalizations. Hear from experts Susan Tolle, MD and Bud Hammes, PhD on how a regional system of standardized medical orders can assure that goals of care are better respected.
