OACIS Home-based Service: Delivering Palliative Medicine in the Home

In this audio conference, you will be introduced to new innovative models and learn how integrating palliative care into clinics and home-based programs are emerging. Services are available in mostly oncology-related clinic settings.  Hospitals and health systems are also implementing new models of care whereby palliative care clinicians make visits to patients in their homes. This presentation will explain the rationale for establishing a home-based nurse practitioner staffed palliative medicine service and describe the clinical, logistical, and operational considerations involved in providing such a service.
