Utilizes the KAER framework to help primary care teams integrate brain health into routine care by supporting conversations around brain health, early identification of cognitive impairment, timely evaluation for dementia, and in-office care planning and connection to community resources.
A Companion to the KAER Toolkit for Brain Health that adapts the KAER framework to support the brain health of adults with intellectual and developmental disabilities (I/DD) by helping health care providers detect changes in cognitive functioning, adapt assessments appropriately, and make informed decisions in partnership with individuals with I/DD and dementia and their caregivers.
Applies the KAER framework to the chronic disease of obesity and help health care providers initiate respectful conversations about weight, assess contributing health risks, evaluate functional and lifestyle factors, and refer to supports—all tailored to the unique physiology and health priorities of older adults.
